Be sure to keep your order confirmation email and order number.\u003c\/li\u003e\r\n\u003cli\u003eYou will be contacted at the email address provided with your order and sent the Mail-In\/Drop-Off Form. If you do not receive the form to your email address (be sure to check junk\/spam folders!) within 2-3 business days, please contact Lydia at Lydia@TheSportsMall.com.\u003c\/li\u003e\r\n\u003cli\u003eFill out the mail-in\/drop-off form that you are sent - be sure to include all of the items you wish to have signed along with appropriate inscriptions and any requests such as pen\/ink color, location, etc.\u003c\/li\u003e\r\n\u003cli\u003eInclude the following in your mail-in package(s): each item you wish to have signed, any pens\/markers if you wish to have a certain type\/brand that we may not have, along with the completed mail-in form. \u003cstrong\u003eIMPORTANT: Make sure all items are labeled appropriately on how you would like them to be signed and what color ink you want to use.\u003c\/strong\u003e\n\u003c\/li\u003e\r\n\u003cli\u003eMail your package(s) to Ohio Sports Group at the address below. \u003cstrong\u003eBE SURE TO KEEP YOUR TRACKING NUMBER!\u003c\/strong\u003e\n\u003c\/li\u003e\r\n\u003cli\u003eEmail a digital copy of your Mail-In\/Drop-Off Form, along with your order information (name, order number, email address, phone number) to \u003cstrong\u003eOrders@OhioSportsGroup.com - \u003c\/strong\u003eplease CC \u003cstrong\u003eLydia@TheSportsMall.com.\u003c\/strong\u003e\u003cstrong\u003e\u003cbr\u003e\u003c\/strong\u003e\n\u003c\/li\u003e\r\n\u003cli\u003eWe will contact you with any questions or concerns.\u003c\/li\u003e\r\n\u003cli\u003ePlease be sure to include your return shipping address on the form. Ohio Sports Group Office drop-offs MUST have an assigned drop-off appointment date and time. Items dropped off at Five Star must be done within store hours. You will be notified of any dates\/times that aren't available.\u003c\/li\u003e\r\n\u003cli\u003e\u003cstrong\u003eIMPORTANT: When dropping off, make sure items are labeled appropriately on how you want them to be signed and what color ink you want to use.
